The United States Department of Labor released the final report on the fatal Randolph County mining accident from August 2017.

According to the report, Owen Jones, 51, fell onto a moving conveyor belt at Pleasant Hill Mine and was killed August 25th. Jones fell when he attempted to complete his examination.

DOL officials say the equipment was not suitable for crossing over on. Additionally, the operator should have installed a guard to prevent miners from crossing the belt in an unsafe manner.

Jones had 27 years of mining experience.
